This includes all the information your insurance company needs to process a claim, dates of service, session codes, fees, and payment confirmation. Just let us know if you need a superbill and it will be emailed to you monthly.

  • Payment is due at the time of service. On the day of your appointment your clinician will charge the card on file. Missed appointment fees are equivalent to the scheduled service. If you miss your appointment that day/late cancel, the price is the same.

Credit cards, debit cards, FSA cards, and HSA cards are all stored securely in your portal account (the electronic health records system), so session time is not utilized for payment.

  • Under the No Surprises Act You have the right to receive a “Good Faith Estimate” explaining how much your care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for any services. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. Crescent Moon Wellness will provide you with an initial GFE before your initial session and then within 24 hours after the session. You can also ask for a Good Faith Estimate before you schedule a session. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 1-800-985-3059.
